As a healer there is nothing worse than a tank that can't keep aggro off the healer - usually it's due to situational awareness, but I've even had it happen because they are so bad at holding aggro I've had to spam heals on the whole group and end up pulling heal aggro. I had that happen once and the tank criticized me for needing to stop for mana after a few pulls! Needless to say rather than responding to him I simply left.
I was in one at lvl 60 something on my shammy where the leader wanted to kick someone for low dps - it was ridiculous we were there to get better gear duh.
The one that happens most frequently however is just getting into a group and having the tank leave - it means a 15 min wait to re queue or just standing around for a long time waiting for the elusive tank to queue up.
